Nursing Home Probe Finds Neglect, Paperwork Issues

(Pandora, OH) A Northwest Ohio nursing home resident spent eight hours in zero-degree temperatures before she died. 

A state investigation found 76-year old Phyllis Campbell had dementia and a history of wandering off at Hilty Memorial Home in Pandora, yet aides never accounted for her whereabouts for about eight hours before staff members noticed she was missing and found her dead from hypothermia two-and-a-half weeks ago. 

The Ohio Department of Health says aides told investigators they failed to do scheduled checks that night, even though they were marked as completed. 

Campbell told an aide she was going home and entered an outdoor courtyard, even though she was wearing a monitor that should have set off alarms, according to the report. 

Four workers put on leave following the incident no longer work there. The results of the investigation are now in the hands of the Putnam County prosecutor.
